Sliding Fee Discount Program (SFDP)
The Sliding Fee Discount Program helps makes healthcare affordable for everyone. This program sets a discount level on what you pay based on the size of your family and your household income (compared to Federal Poverty Guidelines). Click here to view 2023 Federal Poverty Guidelines.

- Primary Care (Medical Care)
- Dental
- Behavioral Health
- Phlebotomy (Labs)
We encourage everyone to apply for the SFDP, even if you have insurance. We understand that it is not always possible for patients to be covered by medical and/or dental insurance, or they may have high deductibles. If you have a financial hardship that is preventing you from paying an outstanding bill or co-pays to Bassett Family Practice, Ridgeway Family Health or MHC Community Dental Clinic, please speak to someone at your clinic’s front desk about waiving the fees. No patients will be turned away for services due to inability to pay.
To determine eligibility, patients are required to:
- Obtain an application from the front desk of the clinic at the time of your visit
- Provide proof of household income or a letter of support at the time of your visit
Once your application is approved, you will be enrolled in the sliding fee program for 12 months. After that your enrollment will expire, and it is your responsibility to renew your application in order to continue enrollment. If your family size or income changes, you may need to reapply sooner.
